Sunshine Health Offering Technology Grants for Rural Health Providers

Sunshine Health is now accepting applications for its Rural Health Provider Technology Grant Program, which is designed to help eligible rural health care providers implement technology-driven solutions that improve patient engagement, care coordination, and health outcomes in rural communities.

Through the program, eligible providers may apply for grants of up to $10,000 to support initiatives such as:

  • Upgrades to core technology systems
  • Integration of new workflows with existing electronic health record (EHR) platforms
  • Technology enhancements to proactively engage high-risk rural patients
  • Training for clinical teams related to technology utilization and patient engagement strategies

Rural Eligibility Requirements

To qualify, applicants must demonstrate that their proposed service area meets at least one of Sunshine Health’s designated rural eligibility pathways, including:

  • CMS Goldsmith Modification – Hospital Only
  • Florida Statute Rural Area of Opportunity designation
  • Florida Commerce Rural Economic Development Initiative (REDI) designation
  • HRSA RUCA Codes/Rural Census Tract ZIP Code eligibility

Applicants must also provide a narrative justification describing how their service area meets the selected rural designation criteria.

Application Components

The application requests information regarding:

  • Organizational overview and service footprint
  • Number of Sunshine Health members served
  • Current technology systems and existing operational challenges
  • Proposed technology project and anticipated impact on rural patient outcomes
  • Alignment with rural health transformation goals
  • Detailed budget and funding allocation plan
  • Measurable project outcomes and sustainability planning beyond the grant period

Applicants are required to identify measurable outcomes and demonstrate how the project will be sustained after grant funding concludes.

Reporting Requirements

Organizations selected for funding must comply with Sunshine Health reporting and documentation requirements, including submitting a summary report within 12 months of receiving grant funds describing project activities, outcomes, and impact metrics.

The application deadline is 5:00 PM ET on Thursday, May 21, 2026.
