Preliminary CY 2025 HHVBP Annual Performance Reports Now Available in iQIES
The Centers for Medicare & Medicaid Services (CMS) has released the Preliminary Calendar Year (CY) 2025 Annual Performance Reports (APRs) for the expanded Home Health Value-Based Purchasing (HHVBP) Model. These reports, available as of September 30, 2025, can be accessed through the Internet Quality Improvement and Evaluation System (iQIES).
CMS recently recalculated performance data for home health agencies in the larger-volume cohort, resulting in modest increases to the Adjusted Payment Percentage (APP) for some providers. Agencies in this group should compare their APP in the preliminary report to the figure in the earlier preview version to determine whether their payment percentage changed. No updates were made for agencies in the smaller-volume cohort.
According to CMS, the CY 2025 APR reflects each provider's performance during CY 2024, which will be used to calculate payment adjustments applied to Medicare Fee-for-Service (FFS) claims in CY 2026. The reports also detail performance across multiple metrics that contribute to the APP, including Improvement Points, Achievement Points, Care Points, and the Total Performance Score (TPS).
To receive a CY 2025 APR, an agency must:
- Have been Medicare-certified before January 1, 2023
- Have enough data for at least five quality measures to calculate both a TPS and APP.
